Méthodologie d’évaluation des substances cancérigènes, mutagènes et reprotoxiques et/ou présentant un effet perturbateur endocrinien dans le cadre d’appels d’offres de dispositifs médicaux stériles

Abstract : Preventing risks related to carcinogenic, mutagenic and reprotoxic (CMR) substances and endocrine disruptors (ED) is a public health challenge. Sterile medical devices (SMD) can be a source of CMR and ED for hospitalised patients. Research on ways to reduce exposure is therefore essential, especially for the most vulnerable people. This includes the choices made by the buying group PharmAuvergne during tender processes. The assessment criteria « CMR/ED » within SMD tenders must allow such substances to be taken into account whilst conforming to the code of public procurement and ensuring the functional aspect of SMD remains at the forefront of the final decision. It is therefore necessary to formalise and validate feasibility for this added criteria. Tools for collecting data and ratings have been created for and tested with 30 SMD suppliers of infusion devices. The results show a large heterogeneity in terms of form and exploitable content to define an evaluation criteria conforming with legal requirements. This also highlights the difficulty to obtain reliable and comprehensive information from the suppliers. Adjustments must be made to the proposed tools for their practical use and collaboration between research team and buyer is required in order to determine the best method of managing specific populations. A scenario has been created to allow the insertion of the CMR/ED evaluation process in tenders and therefore improve the visibility of the issue and promote dialogue between the hospital and supplier.
