Représentations des femmes nullipares à la Réunion sur la contraception par dispositif intra-utérin

Abstract : Background : Reunion Island is composed of a young population in a situation of social precariousness, poor education with a premature sexuality and a greater recourse to abortion than in mainland France (21.8 abortions per 1000 women at Reunion Island in 2018 against 15 abortions per 1000 women in mainland France). The most concerned age group is between 20 and 29 years old, the majority of young women resorting to abortion while using a contraception means and particularly while using the pill which is the most frequently used contraception means among these women. Despite a good health offer on the island, the use of intrauterine devices by nulliparous women is still low even though this contraception means is twice as much efficient as the pill in current use. The objective of this study was to identify the obstacles to the use of intrauterine devices among nulliparous women inhabitant of Reunion Island, to enquire on the roots of prejudices and suggest some solutions in order to develop contraception at Reunion Island. Method : It is a qualitative study carried out through semi-directive interviews of adult nulliparous women at Reunion Island. The data analysis was inspired by the anchor theorization. Results and discussion: The study was carried out between August 2019 and October 2019 among 25 adult nulliparous women at Reunion Island. The use of intrauterine device was determined by the level of knowledge as regards contraception, which was linked to the socio-professional level or the experience of women due to their age or their contraceptive experience. The women belonging to a high socio-professional class or having a good knowledge as regards contraception hightligted the advantages of intrauterine device whereas young women belonging to low socio-professionnal classes brought only obstacles concerning the use of this contraceptive method which were mainly subjective and spread by the friend and family a priori. Conclusion : The main obstacles identified in our study concerning the use of intrauterine devices were the lack of knowledge about this contraceptive method and the overestimation of the pain during the fitting. In order to overcome these issues, the general practitioner and the education system have a major role to play about the information received during the adolescence. In order to encourage this contraceptive method, it seems important to improve the conditions of the fitting by increasing the information about the gesture, by using the lateral decubitus position which is more respectful of the women’s intimacy, by using hypnosis, meopa or diversion during the fitting and by using the direct method also called « in torpedo » which is quicker and less painful for young women.
