Validation et évaluation de la version française du questionnaire de dépistage du handicap auditif HHIE-S (Hearing Handicap Inventory for the Elderly-Screening) chez l’adulte de plus de 60 ans

Abstract : Introduction: HHIE-S (Hearing Handicap Inventory for the Elderly - Screening) inventory is one of the most diffused tool for the hearing loss screening in the elderly. Our objective was to validate our French version by testing it in screening conditions. Methods: we translated into French the HHIE-S inventory respecting the cross-cultural adaptation process. We proposed our translation, composed by ten questions, and scored from 0 to 40 points to people without any otological history, aged 60 years old minimum. Personal and medical history were obtained. Then, pure tone audiometry, speech reception threshold and speech-in-noise audiometry were performed. Subjects considered hearing impaired if the average hearing loss at 500, 1000, 2000 and 4000 Hz was greater than 20 dB HL in one or both ears. Results: we tested 294 subjects (age = 67,3; SD = 6,5 years old). Hearing loss prevalence was 34,7 %. Cronbach’s alpha (test reliability) was high (0,84). Using the score > 8 at the HHIE-S inventory as a cut-off point in identifying a hearing loss, we found a sensitivity of 80,4 %, a specificity of 85,4 %, a positive predictive value of 74,5 % and a negative predictive value of 89,1 %. Theorical hearing rehabilitation concerned 73 subjects (24,8 %). A score > 16 detected them with the best accuracy (88,8 %). Conclusion: our study validates the French version of HHIE-S inventory. This tool could be useful in a program of screening the age-induced hearing loss in the French elderly population.
