Détermination de la différence minimale importante (DMI) du questionnaire LCADL (London Chest Activity of Daily Living scale) pour des patients BPCO sévère à très sévère suivant un programme de réhabilitation respiratoire : analyse post-hoc

Abstract : The LCADL measures the impact of breathlessness on activities of daily life. If its reliability, sensitivity and validity in French were established for patients with severe to very severe COPD, the minimal important difference (MID) for this scale remains unknown. The aim of this study was to determine the MID for the LCADL in patients with severe to very severe COPD undergoing a pulmonary rehabilitation program. Method : we realized a post-hoc analysis from outcomes of 77 patients with severe to very severe COPD who completed a 4 weeks pulmonary rehabilitation program, deriving from a study conducted by Beaumont et al. The MID estimates were calculated from distribution-based method, using the standard error of measurement, the standard deviation and the minimal detectable change. Results : there was a decreased of dyspnea in the current activities of daily life measured by the LCADL (28,2 ± 10,3 à 21,4 ± 9,6 ; p < 0,0001). The MID estimated were 5.07, 4.12 and 11.41 according to the distribution-based method. Correlations between changes in LCADL and in other tools were to weak to use the anchor-based method. Conclusion : results suggest that a change of 11 points in the total score of the LCADL can be interpreted as a meaningful change in patients with severe to very severe COPD undergoing a pulmonary rehabilitation program.
