Profil d’effets indésirables des antagonistes R-NMDA : analyse de clusters des signaux de disproportionnalité extraits de Vigibase®

Abstract : NMDA R antagonists are a class of drugs widely used to treat various diseases such as pain. Currently, only four drugs are on the market. However, their side effects, particularly psychodysleptic limit their development. Our cluster analysis identified adverse events that delineate the typical safety profile of an R-NMDA antagonist. This includes expected psychiatric and neurological disorders, such as hallucinations or deliria but also arrhythmias, blood pressure disorders, respiratory failures, micturition, vision, musculoskeletal and gastrointestinal disorders. On the contrary, adverse reactions specific to one or two drugs have been identified. There is a risk of abuse and dependence but also of immuno-allergic effects with ketamine and dextromethorphan. Amantadine and memantine have in common frail elderly-related adverse drugs reactions. Finally, the hepatobiliary and uro-nephrological damage specific to chronic misuse of ketamine, as well as some specific adverse effects of amantadine (livedo reticularis) and dextromethorphan (acute generalised exanthematous pustulosis) have been identified. The innovative approach of this method is based on the hierarchical classification of adverse reactions by an algorithm (data clustering). However, due to the many biases inherent in the methodology used, further investigations are needed.
