Objectifs et contenus essentiels de la consultation de la deuxième semaine du nouveau-né

Abstract : Context: A medical check-up two weeks after birth has been recommended for newborns by the French High Authority of Health (HAS) since 2014. It became a compulsory health examination in 2019. Objective: To offer less experienced primary care professionals (general practitioners, young pediatricians) guidelines to the essential content and objectives of this consultation. Method: The expert opinion was solicited by a questionnaire in two rounds, aimed at approaching a consensus according to the Delphi methodology (HAS criteria). Between the 2 rounds, they received a summary of their personal responses, aggregated responses and comments from the group. The experts solicited were perinatal professionals as well as parent representatives of users. Experts only answered questions for which they felt their opinion was legitimate. Results: A panel of 67 experts was made up of 22 pediatricians, 13 liberal general practitioners, 9 PMI (maternal and child protection) physicians, 10 midwives, 7 children’s nurses, 2 psychologists, 1 child psychiatrist and 3 patient-experts. 8 of the 208 proposals in the questionnaire obtained strong agreement at the end of the 1st round and 35 at the end of the 2d. 16 of the 30 proposals with relative agreement obtained a strong consensus among pediatricians. The consultation objectives that obtained strong consensus focused on the physical health of the newborn, monitoring and adjustment of feeding, screening for difficulties in the parent-child bond. Conclusion: This work will make it possible to offer consultation support for general practitioners and young paediatricians that will be more complete than the content suggested in the health record. It will also allow to build training programmes. The consideration of environmental aspects in the broad sense was facilitated by the methodology used, calling on a consensus resulting from multiple perspectives.
