Discrimination perçue et survenue de symptômes psychotiques dans les groupes ethniques minoritaires : une revue systématique de la littérature et méta-analyse

Abstract : Schizophrenia is defined as a particularly severe psychiatric disorder whose etiopathogenesis is currently unknown. The origin of this disease would seem to depend on several factors, particularly sociological. At the same time, there has been evidence of an increased risk of schizophrenia in certain social groups, including ethnic minority groups. We conducted a systematic literature review and meta-analysis to assess the impact of perceived discrimination on the incidence of schizophrenia in ethnic minority groups. This review was based on the PRISMA method, the included studies were obtained from the following databases: Medline, Psychinfo and Web Of Science. Of the 1,209 studies, 17 met the inclusion criteria of our study and 8 were used to conduct the meta-analysis. This work shows a significant positive correlation between perceived discrimination and the occurrence of psychotic symptoms among ethnic minorities with a combined Odds ratio of 1,726 (95% CI [1.044-2.852]) after socio-demographic adjustment. It was also shown that the effect of discrimination on the occurrence of psychotic symptoms was not related to ethnicity. Finally, an interesting finding was that the effect of perceived discrimination could depend on other social factors such as ethnic identity, social support and acculturation style. One of the main limitations of this study is defined by the cross-sectional character of the majority of studies which does not exclude a potential reverse causal mechanism. In conclusion, these findings suggest that discrimination perceived by members of ethnic minorities could help explain an increased risk of schizophrenia in these communities.
