La réalisation de l'échographie pré-IVG dans la prise en charge de l'IVG médicamenteuse et instrumentale

Abstract : Nowadays, ultrasound appears to us simple and without consequence, but its use can raise questions. This is why it is at the heart of ethical and psychosocial reflections. In gynecology, we know little about its use, but it is regularly used during consultations, especially before performing an abortion. This is why we investigated the place it occupies and the way it is carried out by health professionals. The aim of the study was to analyze the use and practice of dating ultrasound for abortion by doctors and midwives, whether or not they perform abortions. To do this, we conducted a quantitative prospective multicentric study on the performance of pre-abortion ultrasound by circulating a questionnaire to all professionals likely to perform it, from November 1st, 2019 to March 1st, 2020. We obtained 228 answers. Our study revealed that about 90% of practitioners felt it was necessary to perform a dating ultrasound before an abortion. In addition, differences in the practice of performing the ultrasound, particularly on the choice of the probe used, but also on the possibility of viewing the ultrasound retransmission screen or differents types of measures during the examination varied between professionals. However, according to our results, similarities in practice were observed regardless of profession and location. In general, practitioners seem to be aware of the difficulty for women of requesting an abortion and are putting in place means to make this examination less difficult and restrictive for women. Nonetheless, the edification and pricing of dating ultrasound in the context of abortion requests is not yet applied by all practitioners performing abortion. The preponderant use of ultrasound for abortion, the differences in practice noted by our survey and the comparison of these practices with women's experiences through the scientific literature, lead to a desire to harmonize the practice in order to provide the best possible care for women. Charters and protocols currently exist but are still too little known and too numerous for women to be equal in the management of their requests.
