Facteurs de risque et complications du diabète gestationnel en Guyane Française, une étude prospective

Abstract : Introduction: Gestational diabetes (GDM) is the most common endocrine pathology during pregnancy. In French Guiana, a region considered to be a medical desert, the prevalence of metabolic pathologies is high and DG screening is systematic, unlike other French départements. This region is characterized by its cultural diversity and the precariousness of its population which impacts the management of this pathology. Materials and methods: The study conducted is monocentric prospective with a collection of data along the water in women aged between 18 years old and 45 years during a period of 6 months (December 1, 2017 to May 31, 2018). The main objective is to analyze the prevalence of gestational diabetes, the social and economic context in French Guiana. Secondary objectives are the analysis of obstetric and neonatal complications. Results: The prevalence of DG in French Guiana is estimated at 10.3%. The study population is characterized by a high degree of precariousness, with 70% of patients receiving universal health coverage or state medical aid, half of whom receive less than 500 euros per month. The study found a caesarean section rate of 32% and pre-eclampsia at 7.4%. The neonatal complications found were 9.3% respiratory distress, 7.4% prematurity, and 2.8% hypoglycemia and 2.8% macrosomia. Maternal-fetal complications are represented at 7.4% by maternal-fetal infections. Conclusion: The DG, whose screening is routine in French Guiana, is very common, in a precarious population, and is at risk of cesarean section pre-eclampsia and severe respiratory distress. Knowledge of population specificities and major pregnancy problems is essential for the prevention of complications.
