Évaluation d'un nouvel appareil de mesure des pressions intramusculaires dans le syndrome chronique des loges d'effort : une étude rétrospective

Abstract : Medical interview and intramuscular pressure (IMP) measurement after exertion help us in the diagnosis of chronic exertional compartment syndrome (CECS). The Stryker® was a useful medical tool to measure the IMP but is not available in France anymore. We present a new medical device, the CompuFlo® from Milestone Medical Company, that could replace the Stryker®. The main objective was to describe data from our current practice in a retrospective study. Methodology: presentation of a bench test to compare pressure measurements between the CompuFlo® device and the PressureMAT® system that is considered the benchmark tool. Then, descriptive retrospective single-­center study realized between january 2018 and march 2020. Data were analyzed from the Dxcare® software program. Direct IMP measurements were done with the CompuFlo® 1 min and 5 min after exertion under echographic control. All patients consulting for suspected CECS were included in the study. Results: the bench test shows measures variations of 1 to 2 mmHg between the two devices. The data collected from our descriptive study between january 2018 and march 2020, were analyzed in 51 patients with mean age 30. 41 patients had an IMP measurement with the CompuFlo®. The anterior leg IMP measurements were positive for the majority of the patients (mean IMP 1 min after exercise= 64,16 mmHg and 5 min after exercise= 53,75 mmHg). The reproducibility of typical exercise-induced pain was more difficult concerning the forearms (mean IMP 1 min after exercise= 23,08 mmHg and 5 min after exercise= 20,72 mmHg). The posterior leg IMP values were inferior to those found in the anterior compartment of the leg, leading us to question ourselves concerning potential differential diagnosis (mean IMP 1 min after exercise= 21,50 mmHg and 5 min after exercise= 20,13 mmHg). Conclusion: the CompuFlo® is a CE certified medical device, used in clinical practice in local anesthesia to measure intratissular pressure and is a secure and reliable tool in our current sports medicine practice to diagnose CECS.
