Facteurs déterminants à l’utilisation d’analogues du GLP 1 dans le diabètede type 2 en médecine générale : enquête auprès de 14 médecins généralistes prescripteurs d’analogues du GLP-1 en Lot-et-Garonne

Abstract : Introduction: diabetes mellitus is a major critical issue. There are many different therapeutic methods, but the reference treatments are still old fashioned. Among the new therapeutic classes available, GLP-1 agonists seem to stand out and are gradually becoming established on the French market. It seems relevant to focus on the determining factors in the use of GLP-1 agonists by general practitioners, who are key players in the management of diabetes mellitus. Method: this is a qualitative study carried out among general practitioners prescribing GLP-1 agonists in Lot-et-Garonne with double thematic coding and grounded theory analysis. Recruitment was carried out by email via the mailing list of the Lot-et-Garonne’s Medical Council and by telephone canvassing from the investigator, in order to obtain a sample with maximum variation. Results: 14 semi-structured interviews were conducted. Physicians were seduced by the efficacy, simplicity and tolerance of GLP-1 agonists, but hampered by the injectable route administration and the risk of adverse effects. Their use of GLP-1 agonists was influenced by drug industry, notably through medical sales calls, and local referrals. To a lesser extent, recommendations of good practice and the opinions of the medical press could influence their use of GLP-1 agonists. Their use of GLP-1 agonists was guided by their experience and took into account patient representations. Conclusion: although the practices of general practitioners in terms of use of GLP-1 agonists are close to an EBM approach and respond to a shared medical decision, they remain strongly influenced by drug industry and local referents and too little by evidence. A training program in the practice of critical article reading and EBM approach seems to be an interesting perspective in the initial and continuing education of general practitioners.
