Étude de l’exposition aux infections cutanées à streptococcus pyogenes chez les patients atteints de rhumatisme articulaire aigu en Nouvelle-Calédonie : étude cas témoins et épidémiologie descriptive dans la province des îles Loyauté

Abstract : Background : Rheumatic fever (RAA) is a major public health problem in the South Pacific. New publications from this region raise the potential role of group A beta hemolytic streptococcal (GAS) skin infections in the genesis of the disease. In New Caledonia, the dispensaries of the Loyalty Islands province have an exhaustive database providing information on the medical follow-up of each patient since 1998. We conducted a multicenter case-control study to compare exposure to GAS skin infections prior to the onset of the disease. Method : Each case was matched to 3 comparable controls on age, gender and location. We studied exposure to presumptive GAS skin infections and angina prior to diagnosis through the use of algorithms examining electronic medical records. Results : Among 311 cases of ARF and RHD, there was overexposure to presumptive SGA skin infections from birth, in the 6 months, year and 5 years prior to diagnosis. The odds ratio is 2.182 (1.461-3.259). Furthermore, they confirm the high incidence of skin infections in this area contrasting with the low incidence of angina. Moreover, a correlation is observed between the seasonality of ARF and skin infections and we notice an overlap of areas with high incidence of ARF and high incidence of skin infections. Conclusion : Our results support the hypothesis that SGA skin infections play a direct or indirect role in the pathophysiology of ARF. In view of these results, there is an urgent need to implement measures to reduce the incidence of skin infections in the Loyalty Islands.
