Les problématiques de mise en place d'une communauté professionnelle territoriale de santé sur le territoire du Sud-Manche

Abstract : Introduction: The Territorial Professional Health Community (CPTS) is a new way of organizing, allowing health professionals to gather in the same territory, around a common medical and medical-social project, with the aim of facilitating the patient care journey while helping to improve the working conditions of professionals. Objective: Determining the problems of setting up a CPTS in the territory of South-Manche. Results: The problems highlighted are numerous and fall within different areas: the project initiative left to professionals with an unstimulating national leadership, determination of a territory of action on a scale population, making a precise territorial diagnosis, organizing and managing a multidisciplinary team, communicating with the players in the territory, developing digital coordination and communication tools with respect medical secrecy, drawing up a forecast budget. Conclusion: The South-Manche CPTS has a central role in the future practice of general medicine and primary care, dealing with new public health issues posed by population change. and care pathways.
