Conception, selon la méthode de consensus Delphi, d’un support d’aide à la prise en soin des douleurs neuropathiques chroniques destiné aux médecins généralistes

Abstract : Background: general practitioners (GP) have difficulties to diagnose and manage neuropathic pains (NP), despite the existence of guidelines. Our objective is to define and write a file suitable for GP’s practice in shape and content, to help them manage patients suffering from chronic pain with neuropathic compounds. Methods: according to the Delphi method, we measured the degree of agreement of 15 GPs by rating scale on a questionnaire’s assertions, during 2 successive rounds and under predefined rating rules. The experts often had to choose between competing assertions. Non-consensual group of assertions after the first round were reworded thanks to the experts’ commentaries, and reassessed in the second round. The final consensus was reached after the second round. Results: after 2 rounds, 31 of 82 assessed assertions were valid, be it arbitrarily, or with strong or relative agreement. We couldn’t conclude on special populations’ assessment tools and medication’s contraindication as the rating and commentaries of the experts were divergent. Conclusions: the final consensus obtained, the experts’ commentaries and guidelines led up to the conception of a guiding file in neuropathic chronic pains’ management for GPs. Such a tool, widely broadcast, could improve NP’s diagnose and management. Future researches would assess our file, its comprehension by GPs, its consultation’s applicability and eventually would compare it with existent tools such as Palliaclic©.
