Étude sur l’utilisation de la neurostimulation transcutanée à visée antalgique chez le militaire douloureux chronique dans le service de MPR de l’Hôpital d’Instruction des Armées Robert-Picqué

Abstract : The prevalence of chronic pain in the general population is 31.7% in France. Chronic pain induces a high consumption of care, absenteeism at work, and leads to disability. The French Army has estimated the number of soldiers injured in theaters of operations at nearly 12,500 since 1993. In order to meet the demands of soldiers with chronic pain, the TENS is increasingly needed during their medical care. It is a non-drug technique that uses the properties of an electric current transmitted through electrodes. It is used to treat chronic pain of various etiologies. The real drawback of TENS is that it is cumbersome to use. The question of user compliance therefore arises, knowing that the 2003 WHO report on therapeutic adherence reports a prevalence of 50% of non-compliance, all chronic pathologies combined. By collecting in the computerized patient file their medico-social characteristics, the characteristics of their pain, the possible side effects, we wanted to measure compliance and try to discriminate the factors predisposing to poor compliance. This study is a descriptive single-center epidemiological study on a historical cohort. 77 files were complete, met our inclusion criteria, and were therefore reviewed. We can say that soldiers with chronic pain are observant 6 months after purchasing the device, then compliance at 12 months reaches the figures for the general population's compliance with a chronic pathology and finally after 18 months, compliance drops sharply below the average. We will retain from this study that the doctor / patient relationship is an ally when prescribing and re-evaluating prescribed treatments, to condition good compliance. Also, the range of portable TENS has grown over the years and could promote compliance.
