La prescripion écrite d’activité physique par le médecin généraliste : une nouvelle dynamique ? État des lieux en 2020 dans le département des Landes

Abstract : Background: the health benefits of physical activity (PA) as prevention or treatment of many chronic diseases are proven by robust studies. The lack of regular exercise in the French population and the frequency of sedentary lifestyle has increased considerably these last years. PA prescription should be used by general practitioners (GPs). Despite the new health law promulgated in 2016 and the development of PA programs, this prescription is rarely used in primary care. Aims: determinate, in the Landes french department, the rate of GPs who used a written PA prescription to at least one of their adult patients during the last six months, and the factors associated with this prescription. Method: descriptive study of practices, realized by phoning on a random sample of french GPs in Landes. This study was followed by a descriptive analysis of the patients who received a written PA prescription. We compared the characteristics of prescribing and non-prescribing physicians in bivariate analysis. Results: 307 GPs were called, 168 were included (60.9 %). 54 GPs (32.1 %) prescribed PA during the last six months. Exercising in a city with a PA program (p < 0.0001) and medical formation (p = 0.007) were associated with PA prescription. 73.1 % of patients who received written prescription were affected by chronic disease covered at 100 % by social security, 42.3 % had diabetes. Only one was considered as precarious person (beneficiary of CMU or ACS). 84.6 % of patients were referred to a structure offering PA, including 53.8 % to a sports association. Discussion: this study shows, compared to existing study, a higher proportion of GPs prescribing PA. It also suggest the positive impact of PA programs, as well as the role of GPs training in this prescription. The apparent exclusion of precarious patients deserves further study.
