Améliorer le taux de rédaction des directives anticipées : consensus d’experts sur un dispositif d’accompagnement du médecin traitant regroupant formation, information et valorisation financière

Abstract : Introduction: there are obstacles to the writing of the advance directives (AD). Solutions to some of these obstacles have already been tested abroad. Our main objective is to obtain from the health representative bodies in France a consensus about the establishment of a plan uniting training, information and remuneration to increase the rate of AD writing. Our secondary objective is to provide new proposals. Method: semi-quantitative descriptive study with modified nominal group including experts from the Conférence Nationale de Santé (organization consulted by the Health Minister) and the french society of support and palliative care (SFAP). Collection of experts’ proposals and assessment of relevance and consensus’ levels for each one. Qualitative analysis of the group meeting verbatim. Results: thirteen experts were included by email from November 2018 to January 2019 and submitted 104 proposals which were discussed at a meeting held in Paris on April 17, 2019, leading to 61 final proposals. Eight experts voted and judged 79% of the statements as relevant. There was a strong consensus for 17 proposals, especially those to emphasize the humanization of training, to provide guidance to reflection by trained people and to implement discussion put together with several health professionals. To not establish a dedicated remuneration for this support was considered inappropriate but there was no consensus about financing method. Conclusion: with this study, we bring consensual solutions and new ideas, easy to be implemented, in order to increase the writing rate of advance directives.
