Attentes des médecins généralistes sur le courrier de sortie des urgences Lapeyronie pour leurs patients gériatriques

Abstract : According to the recommendations from the French Society of Accident and Emergency Medicine, the Code of Ethics and the Public Healt Code, the doctor of any patient must receive a letter in a short and reasonable period. Indeed, to allow the doctor job throughout the treatment process and for its continuity; he must be informed of the patient care. The aim of this study was to configure a discharge summary from the CHU Lapeyronie for the geriatric patients, to respond to the region’s doctors’ expectations. To answer it, we have made a thematic qualitative study based on eleven individual interviews, semi-directive, carried out on family doctors from april to july 2019. The communication between the city and the hospital is often questioned by the differents actors of the treatment process. The elder people especially, with no letter, can not always notify their doctors the patient’s care they were offered at the emergency service. In case a patient goes back home, the primary care physicians are asking for a post of synthetic discharge summary, received in a short period, in order to optimise the continuity of patient’s care. In fact, a letter received to late can lose its interest. This discharge summary most include the essential elements for the great medical care of the patient like the motive of consultation, exams done and its results, but also the treatment made or the opinions solicited with the specialist doctors. Finally, it is necessary this discharge summary remind the appointments to take or the exams results to get. With the dematerialization of the patient’s data which being ordered more and more in the medical industry, the transmission of a post by secured messaging service seems to be a tool to improve the communication between the primary care physicians and the hospital field, but also to improve the care’s continuity.
