Évaluation du risque de mésusage et de dépendance lors de la prescription d’antalgiques opioïdes pour des douleurs chroniques non cancéreuses : étude qualitative auprès de 12 médecins généralistes bas-normands

Abstract : Introduction: Opioid addiction is a growing crisis in the United States, making it a leading cause of injuryrelated deaths. Although there are safeguards in place in the French healthcare system that have prevented a similar opioid crisis, France has seen an increase in the use of prescription opioids over the last few years that raises concerns and calls for vigilance. The objective of this study was to explore the way general practitioners (GP) assess the risk of opioid abuse when opioids are prescribed for treatment of chronic non-cancer pain. Methods: Semi-structured interviews and focus-groups were conducted and then analysed by thematic analysis. Results: GPs do know the risk factors leading to opioid abuse but don’t use standardized tools to assess them and don’t mention dependance as a potential side effect. Limited, controlled and legitimate prescriptions tended to reassure GPs. They thought that opioid risk assessment tools should be used more often and that patients would benefit from education on opioid abuse. Furthermore, a decrease in opioid prescriptions thanks to non-drug pain treatments could be a good strategy to prevent opioid misuse. Discussion: GPs’ perception of opioid risk assessment could be improved with training in early detection and intervention for addiction issues.
