Gestion de la douleur nociceptive des patients en soins palliatifs par les médecins généralistes : évolution de l’outil PALLIACLIC

Abstract : Introduction: Ambulatory palliative care, more and more frequent, can be difficult for general practitioners. PALLIACLIC is a medical decision support made for general practitioners, on which are references different symptoms that may occur. The page about nociceptive pain was not developped yet. Material and method: The "nociceptive pain" page mockup was made based on a narrative review of literature, then it was submitted to an evaluation by an experts group, with the DELPHI method. Results: Eighteen doctors, general practitioners or hospital practitioners working in palliative care or pain services were asked to take part to the study. Fourteen accepted. At the end of the first and second survey rounds, eleven answers were obtained, and eight after the third round. After the three rounds, 32 of the 35 initial proposals get consensus. Discuss: the 3 proposals which did not get consensus were about management of breakthrough pain. Three topics get consensus but have been debating by the experts: classification of analgesics according the WHO three steps ladder, step 2 analgesics indications in moderate pains, and pain assessment. Conclusion: This evaluation by DELPHI method made it possible to validate part of the mockup, and highlighted the points of reflection to be clarified before its publication on the internet, in order to best correspond to the expectations of general practitioners.
