pH au scalp foetal pré-pathologique : analyse des pratiques professionnelles et impact materno-foetal à propos de l'expérience au CHU de Caen

Abstract : Objective: Our objective was to take stock of the management of a pre-pathological pH scalp during labour in our hospital. Materials and Methods: Our study was retrospective, with a data collection of 112 patients. The first part consisted in comparing fetal and maternal characteristics between two subcategories of our population, defined by the conduct to be followed after the first pre-pathological fetal blood sampling: an expectant group and a delivery group. Analysis : 55 patients were in the expectant group, against 57 patients in the delivery groupe. The arterial pH at umbilical cord at birth was similar to 7.19 for both groups. A lower Apgar score at 5 minutes was noticeable in the expectant group. After a multivariate analysis, some factors may have been related to management: scarred uterus, stage of labour and induction/spontaneous labour. Conclusion: No decision criteria in front of a pre-pathological pH could be objectively extracted by this study. In the same etablishment we found a great heterogeneity of practices, with action to be taken correled to the obstetrician’s experience. However, there was no evidence of any negative impact on either the mother or the child. It would be interesting, however, to continue this study with an analysis of the files by experts of the department, in order to propose an algorithm or a score to help the management of a scalp blood sample, and to formalize the management of these pre-pathological pH.
