Évaluation du maintien des objectifs tensionnels individualisés par de la Noradrénaline concentrée à 10 µg/ml en perfusion continue sur une voie veineuse périphérique

Abstract : Introduction: intraoperative arterial hypotension increases postoperative cardiac, renal and neurologic complications and mortality. The infusion of Noradrenaline (NA) at 10 µg/ml on a peripheral venous route (VVP) would achieve blood pressure (BP) goals. The objective of the study is to evaluate the efficacy of diluted NA by time spent below predefined individual blood pressure goals as a function of baseline PA (anesthesia consultation) and/or PA at carotid clamping.
Materials and methods: the trial is mono-centric observational prospective that included patients in 2018 operated on carotid, aortic or digestive surgeries. The primary endpoint is the percentage of time spent below 10% of set PA targets. PA's goal was left to the discretion of the anesthetist. The complications were also collected.
Results: 194 patients were included (carotid surgery n=142, aortic surgery n=25 and digestive surgery n=27). The average infusion of NA was 130 minutes (± 102). The percentage of AP time below 10% of targets was 8.5% on average, or 9.4 minutes (± 11). The mean hypotension defined by an average BP of less than 60 mmHg was 5.54 minutes (± 6.7). Complications included 25 patients (13%): 6% renal failure (n=11), 3% venitis (n=5), 3% cardiac complication (n=6) and 2% respiratory complication (n=3).
Conclusion: the NA diluted at 10 µg/ml and administered on VVP makes it possible to obtain the defined objectives of AP, without any side effect.
