Prise en charge de patients au décours d’une anaphylaxie : analyse des données du Réseau d’Allergovigilance des patients bas-normands sur la période 2017-2018

Abstract : Anaphylaxis is a systemic hypersensitivity reaction that may require treatment with adrenaline and need referral to an allergist. The objective of our study was to describe post-anaphylaxis care in Basse Normandie Region from January 1, 2017 to December 31, 2018. 303 episodes of clinical and biological suspicion of an immediate hypersensitivity reaction have been declared to the Allergo-Vigilance Network. 236/303 patients (78%) were contacted to assess their allergic follow-up. The suspected cause of the reactions is iatrogenic (74%), venoms (13%), food (11%), unknown (2%). The reactions are mainly of grade 3 said severe (47%) and of grade 2 said moderate (31%) (grade 1 = 19%, grade 4 = 3%). 111/236 patients (47%) consulted an allergist. Severe forms, cases of food allergy, or those managed in the Manche department consulted more frequently. Patients with low to moderate grade reactions (due to patient refusal) and those in Orne department (because of the lack of an allergist) consulted less. The diagnosis of allergy with identification of the allergen was made for 87% of the patients who consulted, for 8% the allergy was not retained and for 5% the allergen could not be identified. Prescription indications for an epinephrine auto-injector are followed at 77%. To improve post-anaphylaxis management, we propose creation a post-emergency allergology consultation within a short time after the episode of anaphylaxis.
