Perception des médecins généralistes quant à leur pratique de la téléconsultation : étude qualitative auprès des médecins exerçant au centre de téléconsultations de Saint-Georges-de-Rouelley (Manche, 50)

Abstract : Introduction: The insufficient medical density in certain territories is a major public health problem in France. To make up for the lack of health professionals, various solutions are being developed in the area. We were interested in a teleconsultation center recently created. It was a consultation room equipped with connected exam tools. The patient, accompanied by a nurse on site, is received for teleconsultation with a general practitioner in the region. Objective: The objective of our study was to collect the perception of general practitioners who have practiced teleconsultations in the center of Saint-Georges-de-Rouelley. Method: It was a qualitative study, by semi-directed individual interviews, with each of the general practitioners practicing teleconsultations with the Saint-Georges-de-Rouelley center. A thematic analysis with triangulation of the data was carried out. Results: All the doctors agreed to recognize the service rendered to the patient and his satisfaction, as well as the importance of selecting suitable reasons for consultation. The presence of a nurse was generally regarded as a real relational contribution, without disturbing the doctor-patient relationship, which remained unchanged compared to a face-to-face consultation. Opinions on the use and quality of connected tools differed. The time-consuming nature was a major criticism. All recognized the importance of adapting to technological change and the increasing place of this type of practice in the future. Discussion: The strengths of our study was the use of the qualitative methodology which allowed a broad exploration of the problem as well as the possibility of collecting data concerning all the doctors of the teleconsultation center. The investigator’s novice experience, the “unicentric” survey, were the limitations of the study. It would also be interesting in order to better evaluate the device to carry out studies with the other stakeholders of the device and to compare the results found for the center evaluated with other telemedicine devices. Conclusions: The Saint-Georges-de-Rouelley teleconsultation center, which has the specificity of a nurse on site, is an experimental device aimed at exploring the possibilities and limits offered by teleconsultation. It is generally perceived as a complementary tool to the usual activity of general practitioners. Many of the results of our thesis were apprehensions that were already present in other studies of doctors who did not practice teleconsultations. So there was little listening from the doctors. An overhaul of this system so that it is more adequate for the work organization of general practitioners is necessary but should be considered in advance with general practitioners.
