La participation en santé en quartiers populaires : quels changements pour les médecins généralistes ?

Abstract : Context: While patient participation in local health care remains rare in France, it is gradually developing in working-class districts, where health-related inequalities are strong. Objective: Explore the general practitioner’s (GPs) changes of practices and perceptions that were generated by participation in local health care in French working class districts. Methods: A qualitative research was carried out with GPs working in centers located in working-class districts where participation in health care is integrated. Semi-structured individual interviews combined with a participant observation at the national meeting concerning health in working class districts were conducted from August to November 2017. This research was part of interdisciplinary primary care research project. Results: 13 interviews were conducted in 9 sites illustrating various forms and levels of participation in health care. A thematic analysis revealed changes in how the centers operated (integration of users in governance, more openness to the district, co-construction of projects). The doctor-patient relationship was also changed: more familiarity, horizontality and as a result more trust. Lastly, the doctors felt a broader knowledge of the users, their environment and their resources. Conclusion: Patient participation in local health contributes to change GP’s practices in working-class districts: concerning the center’s operation, the construction of projects, in the doctor-patient relationship and in the perception that GPs have of inhabitants.
