Quel modèle de prise en charge pour les femmes enceintes en situation de vulnérabilité sociale, en milieu urbain ? Méta-synthèse de la littérature

Abstract : Background: Pregnancy monitoring for women in situations of social vulnerability is often chaotic. Maternal-fetal complications are more frequent in this population and obstacles to access to multiple care. The objective of our study was to identify the elements necessary for effective care of vulnerable pregnant women in urban areas. Method: We performed a meta-synthesis by following the 7 steps of meta-ethnography as described by Noblit & Hare. A systematic review of the literature was carried out by querying 6 databases. An inductive thematic analysis was then carried out.Results: A total of 1,829 references were retrieved after elimination of duplicates and imported into the COVIDENCE software. After selection, 13 articles were finally included for analysis. The thematic analysis highlighted the need to mobilize the capacities of patients by promoting the development of their self-confidence and the acquisition and reinforcement of skills. This was made possible by the establishment of a specific system considering the patient's life context and organized around four main axes which are: the establishment of a relationship of trust, availability, a multidisciplinary approach, as well as a patient empowerment goal. Conclusion: Multidisciplinary care based on a relationship of trust with the aim of empowering patients, through peer support or antenatal care groups, for example, seems to be an effective model for monitoring vulnerable pregnant women.
