Suivi respiratoire des enfants nés grands prématurés : revue systématisée de la littérature et enquête nationale auprès des réseaux de suivi en France

Abstract : Introduction. Children born very preterm today have a better survival rate, however they are still more at risk of developing morbidities, notably respiratory ones such as bronchopulmonary dysplasia (BPD). In France, perinatal care networks (PCNs) oversee the follow-up of very preterm children. We’ve assessed French PCN’s current follow-up practices and compared them to the available recommendations in the literature in order to determine the best respiratory follow-up modalities. Material and methods. We’ve conducted a systematic review of the literature from the PUBMED, CHOCHRANE and GOOGLE databases to find recommendations on the respiratory follow-up of very preterm children. We’ve also sent a national online survey to the French PCNs. Results. The 3 guidelines publications from the literature review unanimously recommend a specific respiratory follow-up as well as systematic lung function testing (LFT). Out of the 24 PCNs, 17 have answered the survey (71%). Most PCNs have a follow-up program with scheduled consultations at specific ages, as well as a respiratory screening consisting of a clinical examination with no LFT. Patients are systematically referred to a respiratory specialist when a BPD diagnosis has been established by 4 of the PCNs. Conclusion. We suggest reinforcing the PCNs’ current respiratory screening by adding systematic lung function tests for all children born very preterm, no matter what their respiratory health history may be. We also suggest that the PCNs refer children with BPD to a respiratory paediatrician after their initial discharge from the hospital.
