Caractéristiques et devenir des patients asthmatiques au décours d'une prise en charge aux Urgences du Centre hospitalo-universitaire de Caen

Abstract : Objectives: Asthma is a chronic pathology and a significant costly health condition, especially through frequent recourse to emergency care. The objectives of this study were: to analyse the consulting population's characteristics, to evaluate the influence of this consultation on the pneumological follow-up, and to identify the recurrence's factors of emergency consultations. Materials and methods: We performed a retrospective analyse of asthmatic exacerbations at the Emergency Department of the Caen's Teaching Hospital, from 01/01/2017 to 12/31/2018. Odds ratio calculations were realised by univariate and multivariate analysis (adjusted for age, sex, initial asthma severity and socioeconomic level, SEL). This notion of SEL was discussed according to the European deprivation index, EDI. Results: 346 patients were included. Only 37.3% of this population was on inhaled corticosteroid. Therapeutic modification (35%) and addressing to pulmonologist (50.3%) were rarely realize after an emergency consultation. The severity of the exacerbation and the initial specialist follow-up emerged as significant factors for induce follow-up in hospital pulmonology. Age is the only risk factor obtained for recurrent consultations. The most disadvantaged SEL was over-represented. Although without significant evidence, low SEL also appeared to be associated with an increased risk of emergency care. Conclusion: Treatment and post-Emergency follow-up protocols could be a way of reducing asthma cost, as could the adaptation of a personalized action plan deliver during pulmonology follow-up in a specialized canter (doctor, advanced practice nurses).
