Suivi gynécologique de prévention en milieu rural : difficultés rencontrées et place de la sage-femme : Étude qualitative auprès de femmes ayant consulté une sage-femme dans le département du Cher

Abstract : Introduction: In 2009 the Hospital, Patient, Health,Territory Act introduced midwifes to a new range of skills allowing them to take part in regular gynecologic exams. The number of doctors and gynecologists keeps decreasing in rural areas making access to health care much harder. Midwifes play a major role in preventive and screening checkups. Methodology: The goal of this research was to study the problems encountered by women in rural areas, the quality of their gynecologic checkups, and their opinion regarding the role of midwifes. We conducted twelve interviews following women whose gynecologic checkups were done by a midwife in the French department of Cher in 2020. Results: The women who were interviewed said the average distance to reach their gynecologist was 40 kilometers. General practitioners played an important role in preventive gynecologic checkups.These women had seen several health professionals for their checkups. The regularity and consistency of these women’s gynecologic checkups were satisfactory. Most women had met a midwife for the first time during their pregnancy and later decided to have her insure their gynecologic checkups. What women appreciated with midwifes was their patient-centered approach and the chance to have a reproductive health checkup throughout their emotional and sexual life. Discussion: Medical specialists deserting rural areas resulted in great distances and long waiting periods which pushed women to see their general practitioner for their routine gynecologic checkup. In spite of problems and a lack of information, the consistency of checkups was similar to the one observed in the rest of France. Midwifes are key in the reproductive health of these women. They can provide local health-care services as well as a global care. Despite the increasing number of independent midwifes their role in certain connected areas still lacks visibility. Conclusion: The role of midwifes seems paramount in rural areas in order to improve preventive gynecologic care and its access.
