Étude rétrospective analytique multicentrique : étude du rapport neutrophiles-lymphocytes comme facteur prédictif d'évolution vers une forme critique de pneumopathie à SARS-CoV-2

Abstract : Introduction: The ongoing SARS-CoV-2 pandemic’s main feature is a viral pneumonia, and ARDS can occur in this setting. Pejorative predictive elements have been identified, such as NLR. Methods: Retrospective multi-center study in Normandy on patients treated for severe SARS-CoV-2 pneumopathy during the first epidemic peak between January and July 2020. A multivariate analysis was performed to identify risk factors of critical form. Results: 149 patients were included, with a median age of 71 years. The most frequent co-morbidities were hypertension (53.0%), diabetes (20.8%), or neoplasia (15.4%). 51.0% of patients had a critical form. The increase in NLR from D1 to D3 was not a risk factor for critical COVID-19 lung disease. Multivariate analysis identified two risk factors: SOFA score at D1 greater than 2 (OR 5.1, p=0.01), severe CT involvement (OR 6.1, p=0.02), and a protective factor: CRP less than 83 mg/l at D3 (OR 0.2, p=0.002). Overall mortality was 23.5%. Multivariate analysis identified three risk factors: severe CT involvement (OR 6.9 p=0.0004), diabetes (OR 3.0, p=0.02), NLR at D3 greater than 8.53 (OR 3.5 p=0.02). Conclusion: Our study failed to demonstrate the dynamics of NLR between D1 and D3 as a risk factor for progression to a critical form of COVID-19 pneumopathy in a population similar to those already described in European studies. Co-morbidities and an increase in systemic inflammatory markers were associated with mortality in our study.
