Prise en charge odontologique du patient en prison : focus sur le projet pilote de la SAS des Baumettes

Abstract : Some health problems in prison are mentioned in the literature, but oral health is underrepresented. However, dental care is the first somatic request in a remand center. The literature describes a very degraded oral health in prisoners. For the past ten years, human and material resources have made it possible to meet demand; but the prison context makes continuity of care difficult. The Exit Support Structure (SAS) of Baumettes in Marseille, France, breaks away from this classic dental care by putting first personalized support and health education on an individual and collective level. The patient and a multidisciplinary team co-construct a medico-social project, which can issue an exit permission from the judge responsible for the enforcement of sentences. The person can go to a doctor in the chosen neighbourhood, which allows continuity of follow-up after his release. This study looks at the population (n = 379) of this pilot structure between January 2019 and March 2020, to identify the key determinants and confirm the relevance of the chosen axes. Addictive behaviors - especially tobacco and cannabis - and eating behaviors appear as the main risk factors. These results validate the choice of health education themes proposed and highlight a significant change (CAO = 8.14) in the oral health associated with a very young population (x ̅ = 28.75 years). Finally, the comparison of oral and dental determinants reveals a statistical association between cannabis exposure, risk behaviors and dental / periodontal degradation.
