Influence du remplissage vasculaire maternel lors des accouchements par césarienne sur la perte de poids des nouveaux-nés allaités : étude prospective multicentrique préliminaire

Abstract : Objective: the main objective of this study was to establish the influence of intravenous fluid administration in caesarean patients through the weight loss of the newborn within 96 hours after birth. Methods: it was a prospective, multicentre observational study which was conducted in five hospitals in the Alpes-Maritimes department in France between October 13, 2019 and January 31, 2020. Inclusion criteria were based on: women who had given birth to an healthy newborn by cesarean section, following a low risk singleton pregnancy, who’s exclusively breastfeeding her child. The main criterion in judging were maternal fluid intake and weight loss of the newborn. Results: 71 mother-child couples were included. Maternal vascular filling hadn’t influence with the weight loss of the newborn in maternity (p > 0.1). The induction of labor and the duration of it didn’t impact maternal VR (p >0.1). A positive correlation between maternal RV and neonatal arterial pH (p <0.1) has been highlighted. 49 of the newborns lost at least 7% of their birth weight. 7 of them lost more than 10%. The absence of labor and fluid’s types did not influence this weight loss. On the fourth day, loss of weight has been confirmed either by female sex newborns or those coming from induction of labor (p < 0.05). Conclusion: This study doesn’t demonstrate a link between the vascular volume injected during labor and an increase in neonatal loss of weight. The identification of the threshold quantity of intravenous fluid for which weight loss greater than 7% is possible in order to put in place preventive measures to improve care for the newborn.
