Radiothérapie interne vectorisée par 177Lu-PSMA-617 des adénocarcinomes prostatiques métastatiques résistants à la castration : à propos de quatre cas et revue de la littérature

Abstract : Purpose: PSMA is a type II transmembrane protein over expressed in prostate cancer especially in high grade tumours, metastatic disease and in castrate-resistant prostate cancer. In phase II trials, 177Lu-PSMA therapy has shown promising results in treating patients with metastatic castration-resistant prostate cancer. The present work summarizes the first French experience with this treatment in metastatic castration-resistant prostate cancer and a short review of the literature on this topic. Methods: four patients underwent 177Lu-PSMA therapy (10 cycles, median activity: 6 GBq). Biochemical response was defined as prostate-specific antigen decrease of 50% or greater. Toxicity evaluation was based on physician's reports and biochemical results. Evaluation of xerostomia was based on patient’s complain of dry mouth and saliva test. Clinical response and assessment of life quality was objectified using questionnaires. 68Ga-PSMA-11 PET/CT and 18F-fluorocholine PET/CT were used to evaluate imaging responses. Results: analgesic control and quality of life were improved in all the patients. 2 patients presented a biochemical response. No CTCAE grades III toxicity occurred. Conclusions: these cases illustrate the safety and efficacy profile of 177Lu-PSM-617 which seems to be a promising therapeutic strategy for metastatic castration-resistant patients. However, randomized controlled trials are warranted to obtain robust data.
