Étude de cohorte rétrospective monocentrique visant à objectiver les inégalités d'accès aux thérapies innovantes des hommes atteints de cancer du sein suivis au Centre François Baclesse entre 2015 et 2019

Abstract : Introduction: Male breast cancer (MBC) is a distinct entity from female BC. With <1% of all BC, MBC remains under studied. The management is extrapolated from that validated in women. The underrepresentation in clinical trials (CT) has led in France to gendered authorizations denying them access to new treatments. The objective of this work was to assess the disparities in access to innovative therapies for men followed for a BC in the Center François Baclesse (CFB) from 2015 to 2019. Methods: Retrospective study including men treated in the CFB for BC regardless of pathology and treatment line between 2015 and 2019. Women meeting the same criteria were quantified. The primary outcome was the inclusion rate in CTs. Secondary endpoints were eligibility rate, progression free survival (PFS) and overall survival (OS). Results: Fifty-eight men were included, and 8053 women quantified. The male and female inclusion rates were 1.7% and 5.8% respectively, with no significant difference (p=0.26). Only 48% of CTs were open to men (p <0.01). All during the treatment, there was a significant difference between the number of CTs available and the number of CTs that would have been available in the absence of gender in the inclusion criteria (p <0.01). The rates of PFS and OS, all stages combined, at 5 years were respectively 61% and 72%. Conclusion: Despite a gender inequality in eligibility for CTs, there was no difference in the inclusion rate of men compared to women.
