La pratique des Thérapies Comportementales et Cognitives par les médecins généralistes du Calvados dans l'insomnie chronique des patients âgés de 18 à 65 ans

Abstract : Chronic insomnia is a real public health issue. Hypnotic prescription, mostly by general practitioners, is the most common therapeutic response. However, the Haute Autorité de Santé recommendations are clear, Cognitive Behavioral Therapies (CBT) are the first-line chronic insomnia’s treatment. The objective of this work was to carry out an inventory of the practice of these therapies by Calvadosian general practitioners in chronic insomnia of patients aged 18 to 65 years by putting it in parallel with the therapeutic expectations of these same patients. We interviewed Calvadosian general practitioners and patients via two questionnaires. We tried to understand the challenges and barriers to CBT practice for insomnia (CBT-I) for Calvadosian general practitioners. At the same time, we interviewed patients about their potential chronic insomnia problem, their therapeutic expectations, particularly regarding their general practitioner, and their opinion about tools to help them better deal with this problem. The results of the questionnaires showed that patients are looking for a non-medicinal management of their insomnia, so general practitioners must develop their treatment towards a validated non-medicinal approach. CBT-I is the perfect answer, but its implementation remains difficult. Thus we were able to make practical proposals to promote their practice by general practitioners, in line with patients' expectations.
