Évaluation de l'état nutritionnel des enfants scoliotiques hospitalisés pour arthrodèse vertébrale au sein du service de chirurgie pédiatrique du CHU de Caen

Abstract : Introduction: Malnutrition, even in children, is a major issue. It increases complications, length and costs of hospitalisation. Scoliosis surgery can cause it or make it worse. The objective of this study was to assess the nutritional status of children with scoliosis undergoing spinal arthrodesis. Material and methods: A retrospective study was carried out in children undergoing spinal arthrodesis at the Caen University Hospital from July 2009 to July 2019 in order to assess their nutritional status before and after surgery. Complications, length of hospitalization and the impact of a dietitian’s intervention were also evaluated. Results: Two hundred and twenty-one files were analyzed. Eleven percent of the children were malnourished when they entered (5% mild, 5% moderate and 1% severe). There were significantly more complications in malnourished children (36% versus 18% -p=0,004). Superior mesenteric artery syndrome has occurred in 20% of undernourished children. The length of hospital stay was increased in malnourished children, although the difference was not significant. Discussion and conclusion: The nutritional assessment in scoliotic children by body mass index may underestimate undernutrition, responsible for increased complications in postoperative, duration and cost of hospitalization. A specific pre- and post-operative assessment is required to strenghten the management of undernourished or underweight children.
