Étude rétrospective évaluant l’introduction d’un protocole de guidance des gestes de réanimation cardio-pulmonaire lors des appels pour arrêt cardio-respiratoire extra-hospitalier au SAMU centre 15 du Calvados : évaluation des pratiques

Abstract : Cardiopulmonary arrest is the most extreme vital emergency. In France, we have 40, 000 out-of-hospital CPAs a year and the survival rate remains low, about 10%. In face of low figures for first aid training in the general population, it seems essential to offer an efficient guiding system to CPR before the arrival of emergency services. Therefore, we have implemented an algorithm helping with CPA regulation at the SAMU 14. Our objective is to assess retrospectively the changes in the time to start CPR teleguided by medical assistants for regulation as well as the appreciation by the regulation teams of this supporting guideline. The study showed that this time got better, from 444 seconds to 409 seconds, with no significance. The time taken to detect the CPA while on the line with medical services increased insignificantly from 312 seconds before implementing the new protocol to 357 seconds after implementation. Looking for an AED was done for 1 patient before implementation and for 5 patients after implementation. On average, it took 848 seconds to send emergency medical services before implementation and 928 seconds after implementation (p= 0.24). Our study does not show significant improvement of the time taken to implement CPR. After the evaluation of the regulation data, we cannot assert that the form of this new protocol is the cause rather than the difficulties of its implementation.
