Évaluation à distance de l’unité d’insuffisance cardiaque du centre hospitalier de Basse-Terre

Abstract : Context: Heart failure (HF) is a common and chronic disease whose prevalence is booming. Since 2010, the heart failure unit (HFU) of Basse-Terre Hospital Center (Guadeloupe) offers a therapeutic education program. The primary objective of the study was to evaluate, six months after passing through the HFU, the hospitalization and death rates for HF. The secondary objectives were to evaluate the evolution of the treatment from the start and until the end of their stay in the unit and to evaluate the evolution of patients’ knowledge between the entrance in the unit and the assessment at 3 months and 6 months (dedicated survey). Materials and methods: An observational retrospective study, before-after type, was conducted on 109 patients who participated in the program between January 2017 and April 2018. These patients had to suffer from heart failure since at list 6 months, had to be clinically stable and be able to follow a week of therapeutic education program. Average age: 61,9 years, average LVEF: 42 %. Results: 6 months after passing through the HFU, 7,3 % of patients died (n=8). Hospitalization rate for HF included death by cardiovascular cause was 14,7 % (n=16). The prescription rate per patient for each drug class was very satisfaying : Béta-blockers 98,2 %, CEI/AIIRA 92,7 %, Loop diuretics 82,5 %, Aldosterone antagonists 36,7 %. At the entrance of the HFU, knowledge score was 1,4/5 ± 1,4, a significant increase (p<0,001) was observed at 3 months: 3,7/5 ± 1,4 (n=75) likeat6 months: 3,8/5 ± 1,2 (n=91). Conclusion: It is difficult to compare the therapeutic education unit (contents and evaluation methods are very different). In front of the numbers of hospitalizations and deaths lower than the national and local averages, the time spent in the HFU of Basse-Terre seems to be beneficial, especially for patients newly diagnosed (low level of evidence). Setting up a sector dedicated to heart failure in Guadeloupe is essential.
