Analyse des appels du centre 15-SAMU 24 par les EHPAD de Dordogne et association en fonction du niveau de médicalisation

Abstract : Context: french population is getting older. To answer the autonomy loss, the government propose special accommodation like nursing homes. Resorting to the emergency services and non scheduled hospitalisation is growing for the elderlies. Pre-hospital medecine and medical call centers (SAMU) are not spared. In New Aquitaine, the department of Dordogne is the second oldest. There are a lot of nursing home for elderly with autonomy loss. The medical call center is situated in Périgueux and receives many calls from it for more or less urgent patterns. Aim: we wanted to analyse the medical answer level (medical advice, private ambulance, firemen, emergency rapid response unit or SMUR) to the calls from nursing homes of Dordogne according to the level of medicalization. Material and method: it was a retrospective mono centric study describing the characteristics of nursing homes’ residents calls and the medical regulation decisions that were made from the 1st of june to the 31st of august 2019. Results: the average age was about 87 years old and most patients were women. The calls were more frequent during day (61,8%). Nursing home with high level of medicalization were more concerned (35,9%). The calls concerned mostly non pathology considered as non urgent. The most frequent pattern was traumatology (25,2%). Liberal practitioners and emergency doctors were equally concerned by the calls. The answers were medical council (35,8%) or sending private ambulance (36,3%). The SMUR was triggered 13 times. There was neither significant difference between the decisions made by the regulator according to the level of medicalization (p=0,4) nor for the other criteria of regulation (priority level, balance sheet by the effector). Conclusion: nursing homes’ level of medicalization is not favoring the use of any medical means by the SAMU but it could be interesting to weigh it according to the number of resident.
