Proposition d’une formation par la simulation sur la prise en charge de la dystocie des épaules adaptée aux besoins des maternités et des équipes obstétricales des Alpes-Maritimes et du Var : étude préliminaire

Abstract : Objectives: to propose a training on the management of shoulder dystocia (SD) adapted to the needs of maternity hospitals in the Alpes-Maritimes and Var.
Methods: prospective descriptive study of 2019 in two stages with an inventory of the needs of maternity hospitals (n=15) followed by an evaluation of the theoretical and practical knowledge of a sample of midwives (10%) on the SD.
Results: 11/15 (73.3%) of the maternity hospitals replied to the questionnaire. 2/11 hospitals (18.2%) drew up a protocol for the management of the SD. 21 midwives were evaluated. In simulation, 52.4% of the midwiwes did not call any of the health professionals when SD was diagnosed. The SD was reduced with birth of the newborn within 3 minutes without sequelae in 33.3% for the Jacquemier and Letellier maneuvers and 47.6% for the Wood Inversé maneuver. Among the specific characteristics of midwives, learning obstetrical manoeuvres through simulation in initial training improves their management, whereas professional experience does not give them better reflexes in the face of the SD. 100% of midwives as well as 72.7% of gynaecology-obstetricians and child-care assistants/caregivers wish to benefit from training on this obstetric emergency.
Conclusion: implementation of training, through simulation, adapted to the needs of maternity hospitals on the management of shoulder dystocia for midwives, gynaecologist-obstetricians and childcare auxiliarys/caregivers within the framework of continuing professional development.
