Analyse prospective par séquençage haut-débit de souches de Staphylococcus aureus d'origine respiratoire isolées au service de Réanimation Polyvalente du CHU de Limoges

Abstract : Staphylococcus aureus is a frequently isolated pathogen in intensive care units (ICU) due to its high abilities to colonize, invade tissues, produce biofilm, escape immune response and secrete toxins. Incidence of low respiratory tract (LRT) infections remains high, especially in patients under mechanical ventilation. Understanding of the virulence mechanisms and resistance spread are key points. Whole Genome Sequencing (WGS) is an innovative tool, which allows the analysis of the bacterial genomic content and allows to link the presence of a gene to the phenotypic characteristics and the pathogenic abilities. This was a prospective observational monocentric study, in which all consecutive S. aureus isolates collected from LRT samples of ICU patients were analyzed using WGS. This study was conducted over a six months period at Limoges University Hospital. A total of 43 isolates were collected from 41 patients. A clonal diversity was revealed with 13 different sequence types (ST) grouped in six major clonal complexes. One isolate showed a new ST, not previously reported and phylogenetically close to ST6 and ST7. Presence of resistance genes, 115 virulence genes and mobile genetic elements (plasmids, phages) were investigated. Those results were correlated with phylogenetic, phenotypic (antimicrobial susceptibility testing) and clinical data. The tst toxic shock syndrome gene, present on the CC30, was associated with a more frequent occurrence of respiratory infections.
