Le confinement, quel impact sur l’entrée aux urgences des seniors non Covid-19 à Fréjus ?

Abstract : Introduction: the lockdown linked to the Covid-19 impacted the French population from the March 17th to the May 11th. The main objective of this research is to assess the impact of this lockdown on the characteristics and severity of the non Covid-19 elderly patients in Fréjus Emergency. Material and method: this is a unicenter retrospective descriptive study carried out at the Centre Hospitalier Intercommunal (CHI) Bonnet de Fréjus-St Raphaël. The impact assessment was based on incidence measurements, causes of admission, diagnoses and severity between 2019 and 2020. A concomitant evaluation of the general practitioner’s mention was performed. Results: 698 and 497 patients aged 75 and over were hospitalized from the March 17th to the May 11th 2019 and 2020 respectively. A drop in the number of patients was noticed between 2019 and 2020. The comparative analysis of the causes of admissions shows a decrease of patients suffering from alteration of the general condition and an increase of patients suffering from anemia, headache, anxiety, depressive syndrome between 2019 and 2020. The comparative analysis of the diagnoses shows a significant drop in the number of patients suffering from parietal chest pain, anguish, dementia, psychiatric disorder and a marked increase of patients with wounds of limbs, head, face, fracture of a limb, contusion drug iatrogenesis and suicide attempts between 2019 and 2020. The severity of patients in 2020 was much higher than 2019. The mention of the family doctor was better informed in 2020 than in 2019. Discussion: our study reaffirms the initial hypotheses : the lockdown resulted in a decrease of incidence and an increase of the diagnostic severity of old people admitted at the non Covid19 emergencies between 2019 and 2020. The comparisons of the motives and diagnoses item by item between 2019 and 2020 enables us to anticipate on the one hand an increase in the number of consultations at the medical office and admissions to emergency hospital department caused by the lockdown and on the other hand raise awareness of vulnerable patients admitted at the emergency for non serious motives.
