Influence de l'éducation thérapeutique sur l'observance et la qualité de vie de patients victimes d'A.V.C. en Guadeloupe

Abstract : Introduction: Stroke is a major public health problem, especially in the West Indies. The objective of this study was to assess the influence of the post-stroke therapeutic education program on the adherence to treatment and quality of life of patients who benefited from it. Methods: Retrospective case-control study of a cohort of stroke patients. All patients enrolled in the ETP program of the Guadeloupe University Hospital from June 2013 to April 2016 were included. Control patients were recruited from another institution that did not propose a post stroke program. A questionnaire (global knowledge, compliance) and 3 scales (Quality of life: SF36, anxiety and depression: HADS and precariousness: EPICES) were subject to of them. Results: 81 patients were included: 33 ETPs vs. 48 controls. The patients were aged 42 to 91 years with an average age of less than 70 years. The threshold for precariousness was reached in both groups (37.3 vs. 36.3) (p=0.658) and anxiety level was limited for the ETP group (8,6 vs. 6,3 for controls). Knowledge of cardiovascular risk factors (p=0.01) and physical activity (39% vs. 19%, p <0.05) was better for patients in the ETP group. It was also easier for them to explain their pathology to those around them (85% vs. 64%, p=0.04) and they practiced less sedentary activities. There was no statistical difference between the two groups overall for SF36 scores, except for mental health, being better for the control group (p=0.007). Conclusion: ETP has many benefits in stroke management. These structured programs should be better valued in the care process.
