Efficacité et tolérance de l'infliximab dans la sarcoïdose : une étude rétrospective bordelaise

Abstract : Introduction: anti-TNF alpha molecules, and especially infliximab, are currently recommended as third line treatments in refractory sarcoidosis. Data are still missing regarding the efficacy and modalities of use of infliximab in this disease. Methods: we collected clinical and biological characteristics from patients diagnosed with sarcoidosis according to the WASOG criteria and who had been treated by infliximab between September 2008 and April 2019 in Bordeaux. Results: a total of fifty-five patients received infliximab : 48 (87%) as second or third line and 7 (13%) as frontline treatment. Complete response was observed in 24 patients (45%) and partial response in 14 (25%) after a median treatment length of 12 months, Overall infliximab led to a response in 70% of patients with a significant decrease in the number of affected organs and tapering of prednisone doses. Infliximab appeared to be particularly efficacious in neurological, lymph nodes, bones, hepatic and pulmonary sarcoidosis. On the other hand, infliximab seemed to have a lower effect in joint and muscular sarcoidosis. We were able to interrupt infliximab after complete remission was achieved in 7 patients, but four of them relapsed within a mean time of 7 months. Infliximab toxicity was important with mostly repeated infectious episodes. Thirty-six percent of patients experienced serious adverse events, leading to treatment cessation in 29% and death in 2 patients. Conclusion: infliximab seems efficient in about 70% of cases in sarcoidosis but with an important toxicity and a high rate of relapse after treatment cessation.
