Dynamisme (en termes d’activité médicale) des investissements publics immobiliers pluridisciplinaires, en soins primaires, des zones déficitaires d’Ille-et-Vilaine

Abstract : Background : In the current context of medical desertification, especially in rural areas, multidisciplinary grouping is playing an increasingly important role in the practice of liberal health professionals. Likewise, local communities are getting involved in this issue by investing in real estate projects intended to accommodate primary care teams. Aim : Exploratory study seeking to assess the dynamism of real estate constructions established at the initiative of local communities between November 2005 and December 2016 in priority areas of Ille-et-Vilaine, intended for primary care teams. This assessment is carried out at least 2 years after their inauguration, through the presence and activity of general practitioners present in these premises. This in order to determine in a pragmatic way the effectiveness of this type of territorial action in the ambient problem of reduced access to primary care in under-endowed areas. Methodology : After a systematic inventory of the structures to be included, information was obtained from their local authority, the doctors installed there and the Rennes 1 medical school. The primary endpoint chosen was the medical occupancy rate of the structures. The secondary endpoint chosen was the total number of procedures reimbursed in 2018 for doctors installed in these structures.Results : 87 municipalities were contacted; 9 structures were included. These structures have been described. The median medical occupancy rate was 50%. The activity could vary between 1664 and 13,057 annual acts with a median of 6,584 annual acts. 2 analysis groups were carried out: structures having reached 100% or more of the medical occupancy rate versus those not filled. Conclusion : More questions than answers resurface in the light of these results. The low power of the study and the questionable relevance of the endpoint attend for the greatest caution in interpreting the data. Nevertheless, the study opens up the possibility of continuing research in many areas concerning this new subject which has not yet been explored in the literature.
