Deuxième étape de la création d’un outil d’aide à la prise en charge des urgences de la main : élaboration d’un consensus d’experts sur le contenu de cet outil

Abstract : Introduction: this thesis follows the initial work of Charlotte Gachet, which showed a great heterogeneity of practices that often do not comply with the recommendations concerning the management of urgent hand problems (wounds, paronychias, blunt traumas) by general practitioners. The objective of our study is therefore to obtain a consensus on the content of a future website on hand emergencies intended in particular for general practitioners and emergency physicians. Materials and methods: prospective qualitative-quantitative, iterative and interactive study according to the Delphi method that includes several rounds and successively propose structured questionnaires to a group of experts in the management of hand emergencies. This group was composed by hand surgeons, general practitioners and emergency physicians recruited throughout France. The degree of individual agreement was assessed using a Likert scale. Then the consensus was established if the median was higher or equal to 7 in the absence of disagreement. A thematic qualitative analysis of open-ended comments was also conducted. Therefore, proposals that did not obtain consensus or that were deemed appropriate in combination with discordant comments were rewritten in the second questionnaire. Results: 46 experts were included from July 2020 to October 2020, including 12 surgeons, 15 emergency physicians and 19 general practitioners. Two rounds were enough to obtain a final consensus for 63 proposals out of the 63 submitted. The analysis of the open-ended comments showed the difficulty in finding common ground between the different specialists in some cases. It also allowed us to highlight possible modifications and suggestions for the future website. Discussion: this study will be used as a basis for the creation of a tool for the management of hand emergencies, which should be free, easy and freely accessible in order to be used in consultations. These results will thus make it possible to launch the elaboration of the website that will have to be promoted and then evaluated in real conditions of use during future thesis work.
