Facteurs influençant le dépistage du VIH, du VHB et du VHC au CeGIDD, chez les patients migrants primo-arrivants d'Afrique de l'Ouest et Centrale orientés depuis la PASS du CHU à Bordeaux : analyse et propositions en vue d'améliorer la prise en charge dans cette population : étude qualitative

Abstract : Introduction: the new migrants should benefit from HIV, HBV and HCV screening tests during a “Health check appointment” within the first 4 months after their arrival on the french territory. To do so, the PASS of the Bordeaux CHU proposes to the migrants to get to the CeGGID. And in fact only a low percentage of the migrants actually participate to the screening tests. Objective: the main objective was to identify the obstacles, the motivating factors and the improvement proposals related to the adhesion to the HIV, HBV and HCV screening tests for the migrants coming from west and central Africa who are prompted to get to the Bordeaux CeGIDD via the PASS of the Bordeaux CHU. Method: 14 semi-structured interviews were done from November 2019 to March 2020. A double coding and a qualitative analysis were then done by the two researchers. Results: a number of levers and of obstacles have been evidenced among the individual factors, the intermediate factors and the structural factors of the determinants of health. Among the factors influencing the adhesion to the screening tests we can highlight: the knowledge and the perception of the screened diseases and of the screening itself, isolation and language barrier, issues with the transportation means and accessibility to the health care centers due to self-organization problems or due to the status of illegal immigrant or due to a fear of the institution, the ignorance of the access rights to the health care system and of its gratuity. Conclusion: many obstacles have been identified and must be resolved to improve the health of this population. The simplification of the intermediate steps and a diversification of the screening means should be developed to optimize the participation to the screening tests. The improvement of the health care literacy and a community approach are other lines of reflection.
