Analyse de l'offre et de l'utilisation des soins thermaux lors des cures à orientation rhumatologique en Auvergne-Rhône-Alpes

Abstract : BACKGROUND: Thermal medicine mainly concern rheumatologic orientation (88% of cures). There is several treatments offered by the nomenclature and multiple programs labeled “Patient Therapeutic Education (ETP)” exist in French spas. It also suffers from an ever-worsening medical deficit. OBJECTIVE: Analyse thermal treatment during rheumatological-oriented water cures. Identify most used treatments on the 6 main rheumatological diseases. METHOD: We make an observationnal retrospective quantitative analysis : using questionnaires intended for spa doctors in Auvergne-Rhône-Alpes, including questions over 5 patients cared during the 2019 thermal season. We also proceed to literature analysis concerning offer of care and FTE in the different spa resorts. RESULTS: 10 treatments represent 78.4% of prescriptions. Several treatments are rarely or not used. There is a faint heterogeneity of prescriptions according to rheumatological pathologies, namely 5 common treatments (aerobic and carbonated bath / massages / collective mobilization pool / shower under immersion in a bathtub / poultices in multiple application) associated with 1 specific treatment for some pathologies (Berthollaix for lumbar osteoarthritis, local ovens for osteoarthritis of the hands, immersion shower in a swimming pool for rotator cuff tendinopaty, jet shower performed by a technician for hip osteoarthritis ). Availability of the most used treatments varies greatly depending on the resort. TVE is poorly used and frequently the validated architecture of educational programs is not fully followed. CONCLUSION: Work on the nomenclature of thermal treatments seems necessary as well as on harmonization of the offer concerning the most used treatments and the TVE programs in the various stations with rheumatology orientation. We finally proposed a prescription aid sheet for rheumatology spa treatments, intended for doctors uncertified in thermal medicine who take care of spa visitors.
