Éducation thérapeutique du patient insuffisant rénal chronique

Abstract : The prevalence of chronic disease is increasing and chronic kidney failure is no exception. Chronic kidney disease evolves in an insidious and silent way, its diagnosis is too often late and its prevalence in the population is underestimated. In France, the number of people suffering from chronic kidney disease is estimated at nearly 5.7 million. Chronic kidney disease affects a growing proportion of the population: in 2018, there were 89,692 prevalent patients receiving replacement therapy. Therefore, early detection of chronic kidney disease must be a priority. Replacement therapy (dialysis, transplantation) has transformed the management of the disease for more than 50 years. Faced with the heterogeneity of treatments and the complexity of the disease and corrective actions, it is essential to consider patients as actors in their management. The latter is therefore accompanied by therapeutic patient education, integrated into the care process. The aim of this thesis is to take stock of the current state of chronic renal failure and to address the implementation of therapeutic patient education programs. Indeed, the objective of this work is to consider the different objectives and corrective measures of personalized approaches in order to establish recommendations for its improvement. The first part is devoted to the scientific approach of the kidney: epidemiology, diseases, mechanisms of action, diagnosis and management. The second part attempts to answer the following question: how can the implementation of a therapeutic education program be beneficial in the case of chronic kidney disease ?
