Don de sang au cordon : facteurs maternels, obstétricaux et néonataux prédictifs d’un prélèvement de qualité (volume, cellularité, devenir)

Abstract : Introduction : The french Agence de Biomédecine has an objective to store 1000 new umbilical cord blood units (UCB) per year, while targeting the quality of the collected UCB grafts. Objective : to evaluate maternal, obstetrical and neonatal factors influencing the quality of UCB (adequate volume, adequate total nucleated cell count and outcome as process or not) collected in utero or mixed collection (in and ex-utero) in a french setting. Methods : observational study on 1520 UCB, from April 2015 until April 2017 in a French tertiary center in the Parisian region. Results : cesarean or instrumental vaginal delivery, high birth weight and female fetal sex significantly increase the probability of obtaining an adequate volume of UCB. Primiparity, instrumental vaginal delivery and low arterial umbilical cord pH significantly increase the probability to obtain an UCB with adequate TCN. Emergency cesarean, high birth weight, low arterial umbilical cord pH and fetal cardiac rhythm anomalies significantly increase the probability of the UCB to be procesed. Conclusion : knowledge of predictive factors of good USP quality is useful in daily practice to target samples most likely to result in a process.
